About Us

Lakeland Care, Inc. (LCI) is a Wisconsin-based managed care organization (MCO) providing long-term care services and supports to eligible frail elders and individuals with physical and intellectual/developmental disabilities through Wisconsin’s Family Care program. Lakeland Care’s goal is to support individuals to live as independently as possible in their homes and communities by providing high quality, cost effective services and supports using a member-centered, outcome-based approach. Currently Lakeland Care supports members living in twenty-two counties and two tribes in Northeastern, Northern and Central Wisconsin and the Fox Valley, utilizing services provided through a network of local businesses plus other community-based supports.
